Thank you for your interest in the First Health network. If you would like to become a credentialed provider and you do not have an established CAQH identification number, please contact Provider Services at 1-800-937-6824. A representative will establish a CAQH account for you to submit your credentialing information online, which is the fastest way to participate in the network and keep your information current.

After you have a completed the CAQH application, please contact Provider Services at 1-800-937-6824 to receive a recruitment packet with the following information:

• Contract with rate
• Client list
• Mailing instructions

Please remember that completing a CAQH online application or submitting a signed agreement does not automatically make you a network participant.

Please remember that sending in an application does not automatically make you a network participant. A First Health associate will send you a contract and rate information. Additionally, there are a number of items that need to accompany the application you submit, so be sure to review the check list to facilitate the contracting process.
